PARTS 1 & 2 - BACKGROUND INFORMATION
Answer
all questions completely in ink, directly on the application form.
Regarding attachments: 1.
If you need additional space for Academic Information, you may
attach a separate
sheet. 2.
Place your name at the top of all attachments. 3.
Attach by paper clip - please do NOT staple. PART 3 - RESEARCH SITE PREFERENCES
Read
the Site Directory 2004 to
learn more about the sites hosting Interns this summer.
If you would like to speak to someone at the site for additional
information, go to the site’s Webpage,
where applicable. Otherwise,
please notify Sherry McCall Ross, who will provide information and/or
a contact person name & number. Select three sites (in order of preference) that are of greatest interest to you. Be
sure to respond to the question regarding living away from home that
appears on the application. PART 4 - ESSAY QUESTIONS
Prepare computer or typewritten answers (double-spaced) to all questions. Answer each question separately, label every page with your name and question number, and attach your responses to the application form with a paperclip (do NOT staple please). Question #1 – Respond separately for each of the three internship sites you select as site choices. Maximum
length: two double-spaced page per
site. Question #2 and #3 -
Maximum length: two double-spaced pages per question.
Your
application and responses will be reviewed and scored by a selection
committee and then sent to the research site where you have been
recommended for placement. It
is to your advantage to provide complete, well thought-out responses to
each question. PART 5 - OFFICIAL TRANSCRIPT Submit an official transcript. A transcript is official only if it is submitted in the envelope provided by the Registrar’s Office with the official seal intact. You may either request that the Registrar’s Office send it directly to Sherry McCall Ross, or include it with your application. PART 6 - RECOMMENDATIONS
1.
Submit recommendations from two individuals.
One must be a faculty member who
teaches
an MCTP course that you have taken or are currently taking (for non-MCTP
students, a faculty member from who you have taken/are taking an education course).
The
other may be a teacher or employer -
current or past.
2. Each recommendation
consists of a letter and the checklist provided in the packet.
3. Enter your name on
the blank line at the opening of the letter BEFORE you give it to
the
individual recommending you. 4. Each
letter of recommendation must be on official letterhead.
5. Writers of
recommendations may send them, along with the checklist, directly to
Sherry
McCall Ross via fax or mail - or you may include them with your
application. If
included in the application packet, the letter and checklist must
be placed inside an envelope
that has been sealed by the writer, with the writer’s signature
over the outside flap PART 7 - COMMITMENT OF TIME
Please
consider the time commitment you are making if you are awarded an
internship,
and sign and date your application as an indication of your
awareness of this commitment. SUBMISSION DEADLINE Complete Application Packets (including transcript and letters of recommendation) must be postmarked no later than March 17, 2004.
