Instructions:

1.  Complete the "Presidential Scholars with Disabilities" scholarship application in Appendix E.

2.  Write an essay that is approximately two double-spaced pages in length.  Describe your background, personal goals and interests, strengths and weaknesses, and any achievements.  Include your education and/or career goals, identifying any field or course of study that you plan to pursue.  Also, describe your disability and how it impacts you academically, as well as any educational accommodations and compensatory strategies that have helped you succeed.  Additionally, it would be helpful to provide an explanation of how the scholarship will benefit you.

3.  Submit documentation of your disability.  Documentation should be up-to-date (i.e., generally within the past three to five years) and completed by a qualified practitioner.  The type of documentation needed depends upon the nature of your disability.  Generally, you should submit a psycho-educational evaluation for learning or cognitive disability, a psychiatrist’s or psychologist’s report for ADHD or a psychological disability, and a typed letter or report from a physician for a medical or physical disability.  (More specific "Documentation Guidelines" are posted on the DSS website in the Student Guide.)

4.  Provide a certified transcript from the school you are currently attending.  Entering freshmen should include their most recent high school transcript; transfer students should include their most recent undergraduate transcript.

5.  Provide proof that you have satisfied Towson University admission requirements for the Fall of 2009.  Applicants should submit a copy of their admission letter from Towson University's Admissions Office.

6.  Completed application packets must be postmarked no later than March 15, 2009 and addressed to:
