Consent for Treatment of a Minor
(To receive a vaccine or any treatment for illness at the Dowell
Health Center, students <18 years of age must have consent from a
parent or legal guardian. Print out the form, have it signed by a
parent/guardian and return to Dowell Health Center. Fax:

New Entering Student to Towson?
Every new student at Towson must complete the university's mandatory immunization
requirements. Go to our secure patient web portal,
MYDOWELLHEALTH, and follow the
instructions. You will still be required to have your
immunization record signed by a health care provider and sent to
us. This form can be downloaded by clicking here:
Take this form to your health care
provider to complete and sign. Either mail, fax, or email a copy to
the Dowell Health Center.

Request for Religious Exemption from Immunization Requirements
(Medical exemptions must be documented by the student's healthcare
provider on the Immunization form and must be vaccine-specific. )
